Completing your pre‑admission details ensures we have all the clinical and administrative information needed before your procedure, helping to streamline your admission and safeguard your care and comfort prior to surgery.

Consultation and Consent
Your doctor will inform you of the date of your admission and the date of your procedure, explain your operation in full and complete your consent form with you.

GP Health Assessment
It is essential that you have seen your GP within the last year before your proposed surgery date, whether or not your surgeon specifically requests a general health assessment.

Insurance Claims
If your admission is covered by WorkCover or a third‑party insurer, please forward complete claim details including your insurer’s acceptance letter to ensure liability is accepted before your procedure.

Language Support
If English is not your first language, please arrange for someone to assist in discussions with our staff prior to admission, so we can provide the best possible assistance.

Carer Arrangement
All patients must have a nominated carer on the day of surgery to accompany them to and from the hospital and remain with them overnight following surgery.

Confirmation and Preparation
Our team will contact you one to five days before your admission to review your completed pre‑admission details and advise you on any preparation or fasting instructions.

Health Changes Notification
Should you develop a cold, illness or suffer an accident such as a fall before your surgery, please notify the hospital or your doctor immediately .

Medication Disclosure
Disclose all medications you are taking, particularly Warfarin, and provide a recent INR blood test result taken a few days prior to admission, as some medications may affect surgical outcomes.
